Case Study: Breach of Confidentiality in Group Counseling

Eugenia is a licensed counselor facilitating a group therapy session for adults struggling with anxiety and depression. The group consists of six members who have been attending weekly sessions for several months. The group dynamic has generally been supportive, with members forming a strong sense of trust and openness. At the start of therapy, Eugenia emphasized the importance of confidentiality, explaining that what is shared in the group should remain private to maintain a safe and trusting environment. All members agreed to this during the informed consent process.

One of the group members, Sam, is a 34-year-old man who has been dealing with social anxiety and feelings of isolation. In a particularly emotional session, Sam disclosed that he had been struggling with thoughts of self-harm but assured the group that he was not in immediate danger and was managing these thoughts through therapy and other coping strategies. The group offered support, and Sam felt relieved after opening up.

A few days after the session, another group member, Lisa, ran into Sam’s coworker at a social event. Without thinking, Lisa mentioned that she knew Sam from their counseling group and casually referenced Sam’s struggles with anxiety and recent disclosure about his self-harm thoughts. Lisa assumed the coworker knew about Sam’s situation, believing they were close friends.

The next day, Sam’s coworker approached him at work, expressing concern about his mental health and suggesting that Sam take some time off. Sam was mortified that such personal information had been shared without his consent. Feeling betrayed, Sam confronted Eugenia at the next group session, questioning how she would handle this breach of confidentiality. Sam expressed that he no longer felt safe in the group and was unsure if he could continue attending.
